I think its a shame that raspberry pi boards dont come with a big red warning on the box that using the wrong power supply could do serious damage and will invalidate any warranty.
It is extremely difficult to break the Pi if you are powering it from the intended usb-c connector. If you are powering it from the gpio pins however, it is quite easy.
So is the problem using the proper PSU or the carelessness of whoever connects wires to the wrong pins and/or shorts them?

Either way, seems very straight forward to me. Apply 5v, never more never less (as measured at the pi side), and make sure the PSU can source enough at least the amount of current your equipment will use + 10% to spare.
